Each year, The State of Nevada conducts an unannounced
survey of all group homes to ensure adequate treatment
for Residents and compliance with all State Regulations
relating to Hygiene, Safety, Nutrition and Abuse.

| The facility is licensed to care for Category 2 residents. There were no complaints investigated at the time of the survey. There were no deficiencies identified during the survey. No further action is necessary concerning this report. Please retain this copy for your records. |
